How do I create a care plan for children?

< All Topics

Care planning is an essential activity for any child within your program, regardless if they’re in family based care or still in residential care.

Each child should have two crucial documents: a sheet that covers their Essential Information and one that establishes their Care Plan. Ideally, these documents should be very detailed. Provide the best information that you can and deliver the content to the child’s primary caregiver. If the caregiver is low literacy, break down this information into very simple, but critical information to ensure safe care of the child.

Document Essential Information

  • THE CHRONOLOGY OF LIFE EVENTS
  • LIFE HISTORY
  • LIFE STORY
  • EDUCATION BACKGROUND
  • HEALTH BACKGROUND
  • BEHAVIOR

  • HOBBIES AND ACTIVITIES
  • FAMILY DETAILS
  • PLACES THE CHILD HAS LIVED
  • THE CHILD’S FRIENDS

  • WHAT SUPPORT IS PROVIDED FOR THE PLACEMENT?
  • HOW WILL THE PLACEMENT BE MONITORED?
